Loss of primary containment counts are the standard tier one and tier two measure, and they are lagging. By the time the number moves, hydrocarbon is already outside the pipe. The tier three and tier four indicators are meant to catch the drift before that — safety critical element failures found on test, overdue inspections, alarms flooding an operator, deferred maintenance on protective systems. They are harder to collect and easier to argue with. Which ones do you report to your board, and has any of them changed a decision?
