Shift handover: what does yours actually capture?

Handover is where a surprising number of incidents begin. The outgoing shift knows the plant is running with a temporary bypass, a pump is noisy, and a permit is suspended rather than closed. How much of that survives the handover depends on whether it is a conversation, a form, or both. A written log without a face-to-face discussion loses the reasoning. A discussion without a log loses everything the moment someone goes home. What does your handover require as a minimum, and what has it missed?
