Reported near misses drop for two opposite reasons. Either the workplace genuinely got safer, or people stopped telling you. The number looks the same on a dashboard either way, and it is usually reported upwards as an improvement. The checks we suggest: has reporting dropped in one crew or across the site, did anything change in how a report is handled, and does the last person who raised one describe it as worth doing. What did you find when the line went flat, and what did you use to tell the two causes apart?
