Near-miss reporting has gone quiet — good news or bad?

Reported near misses drop for two opposite reasons. Either the workplace genuinely got safer, or people stopped telling you. The number looks the same on a dashboard either way, and it is usually reported upwards as an improvement. The checks we suggest: has reporting dropped in one crew or across the site, did anything change in how a report is handled, and does the last person who raised one describe it as worth doing.
