Document control when everyone works from their own copy

The controlled copy is in the system. The copy people actually use is the printout in the drawer, the file on a laptop, or the photograph of page four on a phone. Revision three goes live and half the site keeps working to revision two. Uncontrolled copies usually exist because the controlled one is hard to reach when and where the work happens. What did you change — access, format, the number of documents, the review cycle — and did uncontrolled copies disappear or just move?
